They had a variety of differences that caused them to become separate settlements, even though they were both settled by those of English origin. These differences included their purpose, their economy, the geography, and demographics. The English’s purpose of coming to the New World had a major impact on the separation of the settlements. Those who chose to come to New England came for freedom of religion. They had a strict code of morality, were organized, and came to stay. They believed in self reliance and independence, which resulted in the Mayflower Compact. In these New England colonies, religion/Puritanism was so important that the number one priority for the settlers at Springfield was finding a minister. Court Transcript of BTKs Confession On February 26, 2005, the  Wichita Police announced that investigators had made an arrest in the BTK serial killer case after taking into custody an employee of nearby Park City, Kansas in a routine traffic stop – bringing to an end an era of terror for the Wichita community which lasted more than 30 years. Dennis Rader,  a city employee, a cub scout leader, and an active member of his church, confessed that he was the BTK serial killer. Here is the transcript of his confession. The Defendant: On January 15th, 1974, I maliciously, intentionally and premeditation killed Joseph Otero. Count Two – The Court: All right. Mr. Rader, I need to find out more information. On that particular day, the 15th day of January, 1974, can you tell me where you went to kill Mr. Joseph Otero? The Defendant: Mmm, I think it’s 1834 Edgemoor. The Court: All right. Can you tell me approximately what time of day you went there? The Defendant: Somewhere between 7:00 and 7:30. The Court: This particular location, did you know these people? The Defendant: No. That’s –(Off-the-record discussion between the defendant and Ms. McKinnon.) No, that was part of my – I guess my what you call fantasy. These people were selected. After I got in the house it – lost control of it, but it – it was – you know, in back of my mind I had some ideas what I was going to do. The Defendant: I thought Mrs. Otero and the two kids – the two younger kids were in the house. I didn’t realize Mr. Otero was gonna be there. The Court: All right. The Defendant: I came through the back door, cut the phone lines, waited at the back door, had reservations about even going or just walking away, but pretty soon the door opened, and I was in. The Court: All right. So the door opened. Was it opened for you, or did someone – The Defendant: I think one of the kids – I think the Ju – Junior – or not Junior – yes, the – the young girl – Joseph opened the door. He probably let the dog out ‘cause the dog was in the house at the time. The Court: All right. When you went into the house what happened then? The Defendant: Well, I confronted the family, pulled the pistol, confronted Mr. Otero and asked him to – you know, that I was there to – basically I was wanted, wanted to get the car. I was hungry, food, I was wanted, and asked him to lie down in the living room. And at that time I realized that wouldn’t be a really good idea, so I finally – The dog was the real problem, so I – I asked Mr. Otero if he could get the dog out. Curb Your Enthusiam an in Depth Analysis Essay In the year 2000, the legendary comedian, Larry David ,created curb Your Enthusiasm. It is a comedy television series produced by HBO about Larry David, starring Larry David himself. He plays himself as a retired comedian producer and writer. The show follows him through his day to day life, which frequently puts him in awkward situations as he often gets annoyed with the status qua and normal social situations. Larry is extremely vocal about his irritations, involving not only him, but the other characters on the show. The show depicts an unflinching, self-deprecating depiction of his life and the lives of his family and friends. The show is called Curb Your Enthusiasm because Larry David feels that most people live their lives by means of false enthusiasm. He thinks they do this in order to give off the impression that â€Å"we are better than you†. Larry also called the show Curb because he did not want his audience to expect too much from the show following seinfelds enormous success. Curb eventually became one of the most critically acclaimed television series, reaping in numerous awards and fans. There are many versatile characters on Curb Your Enthiusiasm, which makes the show extremely dynamic. His wife and eventually ex wife, Cheryl, played by Cheryl Hines is usually Larrys voice of reason. She often guides Larry in the right direction of the social norms, and vociferously expresses her annoyance with him on his many social faux passe. Larrys best friend, Jeff Greene, played by Jeff Garlin, is another major influence on Larry’s character. He is known to be without morals and scruples, paying little regard to the fact that he is married and with a child. Many times, Jeff gets Larry involved in his infidelities, constantly building to Larrys own drama. His wife, Susie Green , played by Susie Essman, is a bully. She is constantly putting down her husband,Larry, and friends, often using profanity and sheer uncouth behavior. Although she has a foul mouth, she walks on the straight path of morality versus her husband and Larry. Last but not least, Larry himself. Larry is an uncivilized, crude, and neurotic individual. He usually has good intentions, but they are blinded by his selfish and stubborn personality. He is not shy about his thical principles and morals, and continuously voices his opinion when a situation does not go his way. Generally, he initiates his situations on petty details, which snowball into major issues and dilemmas. He pursues what he perceives to be the right path, although the social norms may dictate something else entirely. His actions continuously lead him into awkward, entertaining, and side splitting situations which he is persistent about correcting. His misjudgments are so extraordinary that Larry David created a place for himself in pop culture termed a â€Å"Larry David Moment†: an extremely awkward social situation. Some of Larry’s most famous indiscretions are â€Å"stopping and chatting† with acquaintances and tipping at restaurants. Larry David himself is an extremely talented comedian. He, along with Jerry Seinfeld co-wrote and produced the mega popular Seinfeld. After nine seasons of Seinfeld , it was an end to an era. Seinfeld ran its last episode in 1998. In 1999, Larry produced the first Curb Your Enthusiasm. Originally It was meant to be a one time exclusive, a one hour mockumentary- when the characters are aware of the precense of the cameras and crew. The show turned out to be a major success. The Larry David show quickly snowballed into Curb Your Enthusiasm, one of the greatest and highly acclaimed shows of all time. Television has many different popular genres, styles, and programs. Before Curb Your Enthusiasm arrived on the scene, the most popular programs from the nineties were Roseanne, Friends, Full House, Family Matters, and Everybody Loves Raymond. These television shows formed TV into new directions and defined the humor of the decade. They were the start of television sitcoms- shortened for situation comedies. A sitcom is a show that features characters sharing the same common environment, such as a home or workplace, accompanied with jokes as part of the dialogue. Originally, sitcoms started as radio shows, but as we evolved to a television-oriented society, sitcoms acclimated as well. One of the most famous original sitcoms is I Love Lucy, known for being one of the best and most popular shows of all time. It was the first show to perform in front of a studio audience. Lucy showed and influenced society in many ways such as the difference between the sexes. â€Å"The I Love Lucy show continued the ever-popular and age-old battle between the sexes scenarios. Ricky and Fred would try to teach the girls a lesson now and then, and vice versa. In many ways the Ricky Fred team vs. The Lucy Ethel team put men and women on equal ground, as the two continually schemed against one another with similar rates of successful schemes and backfired ones. This was one way for Lucy to escape the submissive housewife image with some defiance of her own. There was a constant desire to outdo the other sex, which perhaps was a signal of the changing times and changing roles men and women would hold in the coming decades†. Sitcoms used single camera filming style, which was more practical given the visual effects used in these shows. This allowed for the careful creation of special effects and sharp editing, features which were not possible with the same finesse in a multi-camera production. Many of these programs were not filmed before live audiences, and featured a laugh track. Jerry Seinfeld, in 1989, created Seinfield, the mega popular sitcom, known to be one of the greatest shows of all time. Seinfield follows the life and creator Jerry Seinfeld, as he lives his life using absurdism- conflict between the human tendencies to seek inherent value and meaning in life and the human inability to find any. The characters of Seinfeld have no morals, hope, ambition, and compassion. Everything they do leads to failure. The show is a show about â€Å"nothing†, meaning there is no climax or moments of pathos for any of the characters. Curb also uses many of the same attributes. The show is based on absurdism, with the characters having little or no morals whatsoever. It is also a show about â€Å"nothing†, following the characters mundane day to day tasks. Television shows have a major cultural affect on society. Friends, for example, was so popular that its influences still affect us till this day. Of example, Rachels haircut is called â€Å"The Rachel†, Joeys catchphrase â€Å"How you doin† and the Central Perk Franchise are just a few of the cultural affects Friends had on our society. It also changed the format of love and family, thriving on the concept that â€Å"all you need is good friends†. The sitcom depicts the idea that we can choose our family and live life in a more unconventional manner. According to pop-culture expert at the University of Buffalo, Friends is one of those rare shows that marked a change in American culture. Most of the Television shows that premiered when Curb Your Enthusiasm came out were complete opposites of it. For examples, shows such as Gilmore Girls, Smallville, Scrubs, and Degrassi were extremely popular. These shows are all drama comedies, scripted, and the fictional opposite of documentary style. Another popular television show is Grey’s Anatomy. Greys Anatomy is a medical drama about a group of surgeons working at Seattle Grace Hospital. The show centers on Meredith Grey (Ellen Pompeo) and her life as a resident at the hospital. In addition to her relationship with her neurosurgeon husband, Derek Shepherd (Patrick Dempsey), and best friend Cristina Yang (Sandra Oh), it also explores her relationships with her peers and the other doctors around her. It is filled with romantic drama and ethical dilemmas that are extremely unrealistic. The characters are also very good looking and impracticable. Curb is unique in many ways that makes it exceptionally hilarious and an instant cult classic. The show is mostly improvised, meaning that there is no real script that the show sticks to. The characters have a storyline, based off of a couple of pages written by Larry David himself. The actors then improvise their dialogue based on the story outline, direction, and their own creativity. Unlike most other popular sitcoms, Curb has basically no writers, no script, and no rehearsal. The characters are raw, and easily believable. They do not dress in high fashions, look like models, or act like people normally would. There is no epic romances or crazy drama. The show is just Larry taking on the social scene in his own unique way. In many episodes, Curb—like its predecessor Seinfeld—tied together apparently unrelated events woven throughout a given episode into an unforced climax that resolves the story lines simultaneously, either to Larrys advantage or detriment. For example, in the episode The Black Swan ( season seven, episode 7) that is exactly the case. In the beginning of the episode, Larry discovers that his mothers gravestone is misspelled and vows to fix it. Larry and his buddies are then seen at the snobby golf club, which has many rules that Larry constantly breaks, including accidently killing the owners swan and other golf member and refusing to tip the waiter. At the end of the show the grave says â€Å"mother of swan killer† and Larry gets caught. The whole show ties together beautifully, with every detail eventually playing a part in the ending. Another example of the show is Every show displays this brilliance, making it hilarious and highly enjoyable to watch. The show also uses a single camera setup. The single camera setup is generally utilized on comedy series that either require or strive for specific shots and camera angles and visual set-pieces. When the potential of the single camera filming style is fully maximized the camera movement, the way shots are composed and edited, and other directorial flourishes, will be as much a source of comedy as the behavior of the characters. The shows asthetics are also plain and simple, making it seem as if its real life and easy to believe. This leads to the style of the show- a mockmentary. Mockumentarys are in the same style as a documentary, yet shot when the cast is fully aware that the cameras and crews are there- like a fake documentary. It is highly improvised, meaning the characters do not memorize a script and just act. They must be creative with the story and put their own â€Å"sparkle† in it. The show is not filled with the typical dramas and unrealistic actors that other leading television series are famous for. It is therefore easily relatable and loveable. Another example using all these elements is from season eight, episide three- The Palestinian Chicken. Larry relishes his role as a social assassin until it becomes a liability with his friends, on the golf course and at a Palestinian restaurant. The story once again has a beginning, middle, and ends which all ties together towards the end of the episode. Another appealing aspect to the show is its depiction of Jewish characters. Vincent Brook stated, Curbs commitment to Jewish identification greatly enhances its storytelling capacity, as it lends greater realism and dimension to the characters and opens the show up to episodes with meaningful Jewish themes. Larry’s character mirrors the â€Å"schmiel† from Yiddish folklore. The schlemiel is usually a comic character whose actions lead to his inevitable downfall, but also stands as a form of resistance to social and cultural values and norms. David Gillota wrote: As a true schlemiel, Larrys failure serves as a direct challenge to the status quo and encourages viewers to question the myriad unwritten rules that we follow in our everyday lives. Gillota also observed: Whereas the schlemiel of Eastern Europe encountered problems that mostly affected Eastern European Jews (such as anti-Semitism and economic survival), Larry encounters problems that affect contemporary middle- to upper-class American Jews, namely, Jewish assimilation, secularism, intermarriage, and, as all of these uggest, the Jews precarious ethnic identity in an increasingly multicultural environment. Alec Berg, one of the writers for Curb and Seinfeld said, â€Å"Structure, structure, structure. Every single thing in a script must advance the plot or define a character more deeply (ideally both, in a hilarious way) or it will die in the edit†. Larry David himself is nowhere near as vivacious as the Larry David he plays onscreen. Larry lives vicariously through his character, acting as a total social piranha wherever and whenever he pleases. He has said in an interview that, â€Å"he is my version of Superman. The character really is me, but I just couldnt possibly behave like that. If I had my druthers, that would be me all the time, but you cant do that. Were always doing things we dont want to do, we never say what we really feel, and so this is an idealized version of how I want to be. As crazy as this person is, I could step into those shoes right now, but I would be arrested or Id be hit or whatever. I find the character to be honest. And honesty comes off as cranky or rude, I suppose. But that character is way happier than I am. Im cranky. Hes not cranky. Id be much happier if I were more like him. He also said â€Å"From the character I play on the show? The only difference is he’s much more honest than I am. He’s brutally honest, and I am not. People seem to think that he’s cranky, I get that. I don’t see him as a curmudgeon. I just think he’s honest, and he gets in confrontations with people because he’s honest. I am not honest. Because of this very delicate, socially balanced world we live in, you can’t talk like that†. He feels as if he is breaking through a barrier, an inhibition that he couldnt do as himself. In conclusion, Curb Your Enthusiasm is not your average Television show. Thanks to the comedic genius, Larry David, the show has become a smash hit success, running a full eight seasons and counting. It has become part of pop culture, establishing its own terms and concepts. The use of mockumentary, absurdism, dry humor, and realism are a major part of the shows charm and success. Filled with quirky and dry humor, Curb continues to break barriers, pleasing and shocking its audience’s episode after episode.

It seems that the perks and other in flight amenities are what may be what makes the difference and will impact any of the airline company’s strategy. The airlines have been trying to come up with competitive ways to differentiate themselves from the competition. Many of the airlines even charge for you check bags also. According to Ethan Davidoff in his article The Cost Behind Checking Bags most airlines only charge $25 for a bag to be checked but there is Southwest Airlines that allows two bags to be checked before charging. With that said in the above mentioned article one can see that you definitely want to travel light because for more than one bag the prices start to increase all the way up to $600. Although Southwest allows two checked bags there are other airlines that allow one free checked bag and most have a weight limit of 50lbs. If one were to travel internationally it may be cheaper to just purchase brand new clothes when you land. Not to mention the convenience of not checking any bags or having to wait to pick it up when you land, etc. So not only have airlines starting charging to check bags but many have stopped providing food, unless the flight is over a given amount of time and you are usually paying for that snack. The above mentioned change in airline airfares has certainly affected many airlines’ strategies. Also as mentioned above it has affected them most specifically in what type of benefits, perks, or amenities that they are willing to offer their flyers. Jet Blue has certainly followed suite and done various things to try to set themselves apart Discuss Jet Blue’s strategic intent. Jet Blue says that their â€Å"objective is optimize our fare mix to increase our average fare and, in certain markets, utilize our network to maximize connecting opportunities while continuing to provide our customers with competitive fares† (jetblue. om SEC filings, p. 31). Like any other company they are looking to cut costs without hurting any of the aspects of their company that sets them apart (i. e. customer service, tv, radio). Jet Blue airline seems to have set its strategy to take advantage of the charges that other airlines charge. They have set themselves apart by offering things on their flights that other airlines either don’t have or charge for. Jet Blue allows the firs t bag to fly free, the second costs $35, and the third costs $75. Although they do not have wireless broadband yet, unlike some other airlines, they do plan to have it in place 2012, and this is so because all of Jet Blue’s aircrafts are being set up for ViaSat satellite wireless which is new and no other airline has. Jet Blue is testing the service for ViaSat and the FAA for testing to be approved for use on airlines. However on the ground in the terminal it does offer free broadband at most airports (http://www. jetblue. com/flying-on-jetblue/onboard/wifi. asp). A strategy held by Jet Blue like that of any business is to improve its bottom line. Jet Blue wants not only to be profitable while reducing costs but they want their customers to be happy and have a good flying experience. Jet Blue has a saying on their site that says â€Å"our standards beat their extras† (http://www. jetblue. com/flying-on-jetblue/). They aim to provide stellar customer service. As a matter of fact according to ehow. com Jet Blue â€Å"was awarded the Conde Nast Traveler ‘Readers’ Choice Award’ for best domestic airline† and â€Å"in 2008, it won J. D. Power and Associates’ ‘Highest Customer Satisfaction Among Low Cost Carriers in North America’ award (ehow. com). More specifically they also state on their website that they have † ranked ‘Highest in Customer Satisfaction Among Low-Cost Carriers in North America’ by J. D. Power and Associates for five years in a row† (http://www. jetblue. com/about/work/). Jet Blue airlines prides itself on its customer service. As noticed from the previous paragraph they have won awards for their customer service. It is no wonder that the company believes in making their customer’s flight experience a pleasant one since the company was started by â€Å"David Neeleman and a number of Southwest Airlines employees in February 1999† (ehow. om). Outstanding customer service is part of their strategy and it is one of their many features that puts them heads above the competition. Discuss Jet Blue’s financial objectives and whether or not the company has been successful in achieving this objective. Before even getting into the financial objectives of the company just reading the SEC report for 2010 really shows proof that Jet Blue has been an extremely successful company over the past ten years. Since they were incorporated in 1998, flight starting in 2000, they have grown from $38. million in net income in 2001 to net income of $97 million in 2010 (jetblue. com). The aforementioned data shows proof of growth and success in lieu of turbulent financial times with rising fuel costs. Fuel costs dramatically rose over the company’s time in business which could have easily put a company under but they continue to strive. Discuss Jet Blue’s strategic elements of cost, organizational culture, and human resource practices and evaluate whether each element provides the organization with a competitive advantage. Jet Blue continues to be competitive with their ticket and baggage pricing. In their 2010 SEC filing they term themselves a â€Å"value airline – based on service, style, and cost† (p. 4). They go on to discuss in the overview how they feel that they continue to provide â€Å"best coach product† and â€Å"reasonably priced optional upgrades† (http://investor. jetblue. com, p. 4). As for Jet Blue’s organizational culture their web site specifically says â€Å"If you’re looking for a job where your career, your passion, and your sense of fun are all important then you’ve landed at the right place. At JetBlue we’ve built an award-winning brand based on delivering value, service, style and comfort to our customers — and to our crewmembers. And, by hiring people who share our vision, we’ve created a place where people enjoy coming to work† (http://www. jetblue. com/about/work/). Jet Blue continues remain un-unionized. Discuss Jet Blue’s strategies for 2008 and beyond and evaluate whether or not Jet Blue will be successful implementing these strategies. Part of their strategy in 2008 has been the same as previous and later years, to remain on time with their flights. They have continued to do well meeting this goal. Jet Blue’s ongoing strategy has been to deliver exceptional customer service and on time direct flights. In 2008 they stated in their SEC filings that they were the â€Å"7th largest passenger carrier† and in their 2010 SEC filings they were the â€Å"6th largest†. Regardless of any strategies that they planned to implement they were obviously successful. The proof lies in their continued recognition by JD Power & Assoc, their net income increases, and their growth to 6th largest passenger carrier in the US. While other airlines are folding, being bought out, or merging Jet Blue is continuing to grow. It is no wonder since they offer outstanding customer service as only part of their strategy and it is one of their many features that puts them heads above the competition. Some of their other features that ranks them high with their customers is the fact that they offer 36 tv channels, XM/Sirius radio, and unlimited snacks and beverages to all passengers at no cost. Kudos to a company for keeping its employees (all non-union) and customers happy.

Introduction As we come into the 21st Century, we find Healthcare is at a crisis level. Every agency is working on healthcare reform from policymakers to the public and private sector, as well as federally funded Medicare and Medicaid. The business of Medicine is greatly influenced by the government (federal, state and local levels) and private health sections that initiate policies. Pay for Performance is a reimbursement method where physicians and hospitals can receive a higher reimbursement for duplicate services based on the fact that they deliver better quality care with better results and outcomes. This payment reform offers initiates intended to improve efficiency, value, and quality of health care (Hood, 2007). If all doctors receive the same dollar amount as a doctor with poor outcomes, then the doctor with great outcomes should receive a little more and there will be patients that do not mind paying a higher deductible for better medical services (Mayes, 2006). Definition for Pay for performance Kimmel (2005), â€Å"Pay for performance is a payment approach used in healthcare that is based on clinical information-driven reform. The fundamental concept is to tie payment to how well providers adhere to practice standards. The practice standards are evidence-based and tied to clinical outcomes. The primary areas of focus are preventive care delivery and disease management for chronic illnesses†. Effects on Reimbursement Pay for performance (P4P) is literally a group of performance indicators that are coupled with an incentive. The performance indicators supports the performance aspect of P4P while the incentive indicator is the pay component. Measuring patient outcomes and understanding the variances that  they have, has in part lead to the increasing rise in how pay for performance reimbursement is looked at. This style of reimbursement allows health plans and employers to pay increasing reimbursements to medical providers that have the better outcomes, give average outcome medical providers a chance to improve, and pay those medical providers with the lowest outcomes the least amount of money or not pay them at all (Cromwell, Trisolini, Pope, Mitchell, & Greenwald. 2011). In order to have a pay for performance system in place, you must decide what domains or areas you wish to track, measure, and reward. Some areas in this domain are clinical process, quality and patient safety, access to and availability of care, cost efficiency or cost of care, cost-effectiveness, administrative efficiency and compliance, adoption of information technology, and reporting of performance indicators. These can be set up as a single performance tracker or a multi-domain performance tracker and the measure needed for improvement, importance, and cost. Performance indicators should be valid, reliable, and informative (Cromwell, Trisolini, Pope, Mitchell, & Greenwald. 2011). The Incentive Schemes reward the performance measures, and is another important part of a pay for performance system. Funding proves to be another important part of this type of a reimbursement system. Types of funding include redistributing existing payments where additional funds will not have to be made and the quality of service is already high; however, medical providers with a lower quality of service will receive lower reimburs ements. Generated Savings and New Money are other sources of funding for performance measures. Generated savings claim that an increased quality of service will generate savings, although there are others who feel that new money should be used to fund the performance system. (Cromwell, Trisolini, Pope, Mitchell, & Greenwald. 2011). Impact of System Cost Reductions on the Quality and Efficiency of Health Care The Medicare Physicians Group Practice (PGP) was the first physician pay for performance model used by the federal government. The PGP believes that higher quality and better cost efficiency could be achieved by managing and coordinating patient care and by engaging in wider choices of care management that are able to improve cost efficiency and quality of health care. Interventions include; chronic disease management, high risk and higher cost care management, transitional care management, end-of-life and palliative care programs. If there were a more  successful payment and delivery method to increase the value of health care and improve quality of care, the cost would grow at a slower pace. The American people would be more likely to purchase health insurance coverage that is affordable and more valuable. (Kautter, Pope, & Trisolini, 2007). More progress toward effective delivery and system reform is one of the key elements to achieving the goals to push expanded coverage. Information technology is one of these key elements and a major part of pay for performance system. Information systems uses electronic medical records and patient registries have been created to improve the efficiency and quality of health care delivery. These type of initiatives that are being tested to see if cost savings are generated by reducing avoidable hospital stays, cutting down on readmissions and emergency room visits, while simultaneously improving quality of care (Kautter, Pope, & Trisolini, 2007). Effect of Pay for performance on Health Care Providers and Their Customers Meredith B. Rosenthal states, â€Å"Pay for performance will not replace the existing payment structure in either system, but it does allow payors to take into account a set of quality indicators, in addition to volume of service (as fee-for-service does now) or the nu mber of covered lives (in the case of capitation). In this view, pay for performance can be viewed as a mechanism to correct some of the distortionary incentives that already exist in the reimbursement system†. Physicians in the United States are paid on a fee-for-service basis. This encourages high volumes of services, where there is no regard to the value of services in regards to a patient. When services are reimbursed more generously than others it allows the payment system to influence additional medical services with a heavy emphasis on procedure-based care. Since the physicians pay is not attached to medical services provided, there is really no direct incentive to provide any services (How Will Paying for Performance Affect Patient Care?. (2006, March). Virtual Mentor, 8(3), 162-165). Effects of Pay for performance on the Future of Health Care Goldberg lists three points regarding the most significant implications of the movement toward paying for quality outcomes. These are that the quality and value become real parts of contractual reimbursement, the differences based on quality outcomes will be more evident grouped with provider tiers,  and quality metrics evolve to outcome-based and chronic disease management (Goldberg 2006). P4P is an incentive-based reimbursement system that rewards the best players. This pay for performance system is currently active in health systems, managed care settings, and private and group physician’s practices. P4P is likely to impact the entire health care environment and will provide yet another opportunity for pharmacy to become an active role player and leader with improving quality and efficient health care. The focus is not on value but on quality and cost. Pay for performance is not a new program, but in the age of informed choice, evidence based medicine, and patient safety, it can become the solution to our current health care dilemma (Pay for performance (P4P): Evaluating Current and Future Implications). Conclusion These pay for performance systems and programs will lead expansion across the United States health care industry in the near future. With the implementation of the Affordable Care Act, there has been a great amount of provision made to encourage continued improvement with quality of care. â€Å"Accountable Care Organizations (ACOs) are groups of providers that agree to coordinate care and to be held accountable for the quality and cost of the services they provide† (James, 2012). There needs to be a consensus as to how much of an incentive will have to be given in order to affect the needed change and how should these incentive’s be paid out monthly, quarterly, or yearly; and how can these improvements be sustained over time. Continued experimentation with the pay for performance model should begin to incorporate monitoring and evaluation in identifying design elements that will also affect outcomes in a positive way. Variations in health care markets should be evaluated and include comparison groups to isolate pay for performance from other types of factors. Pay for performance has some great attributes to it and could definitely be the beginning to improvements in quality of service. If physicians are receiving patients and referrals based on their ability to provide quality of service with reduced readmissions and more satisfied consumers, then the care they take in providing services to patients from admission to discharge will create positive change.
